Thank you for your interest in adopting a Korean rescue dog. Please complete the following form to submit your adoption application. We respect your privacy. The information you provide in this application will not be shared outside of the organization.

Please note that we only accept applications from the following cities. Applicants from cities not listed will only be considered at the sole discretion of the adoption team.

- I believe crate training is a great tool for house/potty training, staying home alone, sleeping, and situations where dogs cannot be supervised.
- I believe crate training can be a valuable training tool for behaviors like anxiety, separation anxiety, hyperactivity, etc so the dog has a place to settle down.
- I prefer to keep my dogs contained in a specific room, or in a playpen.
- I don't believe in crate training. I prefer my dogs to have free roam of the home.
- I don't know much about the advantages/disadvantages of crate training. I feel it would depend on the dog.

- I believe with proper recall training, off leash is appropriate.

Adoption Agreement Terms and Conditions
To complete your application, you must review and agree to all adoption terms and conditions. Please consider each item carefully before submitting your application.
Adoption Agreement Terms and Conditions:*
- I verify that all statements made in the adoption process are true to the best of my knowledge. If it is discovered that any statement(s) I have made are found to be false, the adoption agreement is void and the dog may be confiscated at the sole discretion of Free Korean Dogs / Free Korean Dogs US.
- I understand and agree that Free Korean Dogs / Free Korean Dogs US makes no guarantees about the dog's temperament and is not responsible or liable for future damage or injury caused by the dog.
- If your application is approved, do you agree to pay the adoption fee: $760 USD/$960 CAD for dogs under 8 years old, $360 USD/$460 CAD for dogs 8 yrs old and older or special needs.
- I understand if Free Korean Dogs / Free Korean Dogs US is unable to transport the dog to me within two (2) months, I will be eligible to request a cancellation of the adoption with a full refund).
- I understand Free Korean Dogs / Free Korean Dogs US maintains the right to cancel the adoption in its entirety at its own discretion prior to the adopter taking possession of the subject animal, with a full refund.
- I agree that the adoption fee is non-transferable and non-refundable once the dog has been delivered to me.
- I understand the adoption will not be finalized without 1) completion of the adoption agreement 2) payment of the adoption fee, which will be required within 48 hours of the signed agreement and 3) fulfilment of safety protocol equipment/requirements.
- I confirm this dog is being adopted solely by me as per microchip registration and applicable city pet licence registration. The dog will not be sold, adopted, or given by any means to another party, including family or friends
- If at any point I am unable to keep the dog for any reason, I agree to surrender the dog to Free Korean Dogs / Free Korean Dogs US.
- If in the event I decide to surrender back to the care of Free Korean Dogs / Free Korean Dogs US, I understand a formal surrender request and $220 USD/$280 CAD surrender fee (non-refundable) will apply.
- I verify I have thoroughly reviewed the safety equipment protocol and agree to follow the safety equipment protocol for a minimum of the first twelve (12) months of adoption.
- [For dogs over 6kg/13lbs] I agree to activate my Tractive GPS within 24 hours of my dog's arrival and before taking them outdoors.
- [For dogs over 6kg/13lbs] I agree to continue to use the Tractive GPS for a minimum of 12 months under a 1 year pre-paid service subscription upon my dog's arrival.
- I agree to register the dog’s microchip number under my name with www.24petwatch.com within 24 hours of arrival and send registration confirmation to (info@freekoreandogs.org).
- I agree to register and purchase a pet licence under my name from my municipal animal services department, as legally required.
- If the dog is lost, I agree to take immediate action to locate the dog. I agree to contact Free Korean Dogs / Free Korean Dogs US and local animal control if the dog is not recovered within 3 hours of being lost.
- I agree to care for this dog in a humane manner, which includes supplying adequate food, water, shelter, attention, and medical care.
- I agree to never physically or emotionally abuse, neglect, starve, or harm the dog in any way.
- I agree to only use positive reinforcement and fear free training methods, and to never engage in the use of aids such as prong, shock, bark, or e-collars.
- I understand and agree I will assume all medical responsibility once the dog has physically been transferred to my care.
- I agree that the dog must receive veterinary checkups on an annual basis, including core vaccinations and flea/tick/heartworm prevention as advised by a licenced veterinarian. I agree to provide a written statement of the dog’s first annual checkup to Free Korean Dogs / Free Korean Dogs US.
- I agree to never keep the dog in a locked crate.
- I agree that this dog is to be a companion animal only, not a guard dog, and that this dog will live inside my home.
- I agree that this dog will not be allowed outdoors without supervision.
- I agree that the dog will wear a name tag at all times, which will include my contact current contact information, both indoors and outdoors.
- I agree that I may not euthanize the dog except in the case of the dog's terminal illness or injury, or old age accompanied by pain and suffering. In such a case I agree to consult with Free Korean Dogs / Free Korean Dogs US before proceeding, and that the euthanasia must be performed by a licenced veterinarian.
- I agree to never leave the dog unattended with infants and children under the age of 13, and agree to never leave the dog unattended with other animals until I am confident of their compatibility for a minimum of twelve (12) months after adoption.
- I agree to send regular written updates on the status of my adopted dog to Free Korean Dogs / Free Korean Dogs US. Update will be sent bi-weekly for the first 3 months and bi-monthly for the remainder of the first year. Updates will include a written summary, photos and videos.
- I give Free Korean Dogs / Free Korean Dogs US permission to phone or visit my home at any reasonable time to assure the dog is receiving proper treatment and care, should concerns be raised at any point in time.
- I agree to update Free Korean Dogs / Free Korean Dogs US of any change in my current home address and contact information (i.e. phone number, email address).
- I agree that I must notify and obtain agreement from Free Korean Dogs / Free Korean Dogs US PRIOR to planning any trips outside North America with the dog(s). Should Free Korean Dogs / Free Korean Dogs US deem the trip as unsafe for the dog(s), Free Korean Dogs / Free Korean Dogs US reserves and holds the right to recover the dog(s) from you upon demand.
- I agree that this adoption agreement may be specifically enforceable by Free Korean Dogs / Free Korean Dogs US through judicial proceedings, including the right of Free Korean Dogs / Free Korean Dogs US to recover the dog due to any breach in the terms of this agreement.
- In the event that I breach this agreement and Free Korean Dogs / Free Korean Dogs US files suit to enforce or defend the terms of this agreement, I agree to pay any court costs and/or legal fees incurred by Free Korean Dogs / Free Korean Dogs US in connection herewith.
